Erectile dysfunction (ED) refers to the inability to achieve and maintain an erection suitable for sexual activity. In the United States alone, about 40 million men suffer from ED. There are currently two main drugs for this condition, sildenafil (Viagra®), and tadalafil (Cialis®). These drugs are known to be effective for the treatment of ED. However, they have some limitations, including the use of high doses, high plasma concentrations, and lack of tolerability.
Despite these limitations, there is a growing body of evidence that these drugs, including sildenafil and tadalafil, have some potential benefits for the management of ED. However, these medications have not been studied extensively in the United States.
Sildenafil (Viagra®) is a phosphodiesterase-5 (PDE-5) inhibitor used to treat erectile dysfunction in men. It was approved by the FDA in 1998 for the treatment of erectile dysfunction in men. However, sildenafil has been shown to be safe and effective for the treatment of ED in men who have not responded to other treatments. It has also been shown to be safe and effective for the management of ED in men with underlying health conditions such as diabetes and hypertension.
The use of sildenafil and tadalafil in the management of ED in men is not new. There have been several clinical trials comparing these drugs. The results of one clinical trial demonstrated that sildenafil was well-tolerated in men with ED. Another study in patients with hypertension and diabetes showed that sildenafil was well-tolerated and was not associated with serious adverse events (AEs). In a meta-analysis, both sildenafil and tadalafil showed significant improvements in patients with ED in the first 6 months after starting treatment. The primary endpoint was a change from baseline in sexual activity that improved after 1–2 weeks, as well as the overall improvement in sexual activity.
